SUMMARY:The Primary Care Behavioral Health (PCBH) Model of Integrated Care
DESCRIPTION:Description: The Primary Care Behavioral Health Consultation model (PCBH) is a psychological approach to population-based clinical health care that is simultaneously co-located\, collaborative\, and integrated within the primary care clinic. The goal of PCBH is to improve and promote overall health within the general population. This approach is important because approximately half of all patients in primary care present with psychiatric comorbidities\, and 60% of psychiatric illness is treated in primary care (Pirl\, Beck\, Safren\, Kim\, 2001). This webinar overs the framework of the PCBH model\, the behavioral health consultant role\, and a day-in-the-life look at integrated care using this model. \nView Resource \n The previously recorded webinar and associated materials are available in the resource library. \nLearning Objectives: \n\nDescribe primary\, secondary and tertiary care levels\nDifferentiate case-based care from population-based care\nDescribe the roles in the integrated team\nDescribe the framework of the GATHER model\nExplain consultation in practice\n\nContent Level: Foundational \nCourse Interaction & System Requirements: This live webinar is fully interactive.
