SUMMARY:From Beginning to End: A Case-Based Experiential Session About Advance Care Planning - Session 3 of 3
DESCRIPTION:View Resource \n The previously recorded webinar and associated materials are available in the resource library. \nSeries Title: “Control Your Future!”: The Promise and Practice of Advance Care Planning \nSeries Description: Only about one-third of Americans complete living wills\, durable power of attorney for healthcare forms\, Psychiatric Advance Directives\, or other types of Advance Care Planning to document their wishes for which treatments they are willing to undergo in the event they become incapacitated. Even when they have living wills\, those documents may not be implemented in an emergency because they aren’t readily available. In this three-part series\, primary care and behavioral health providers will learn behavioral strategies for engaging even reluctant patients in Advance Care Planning and uploading the resulting documents to the health information exchange through CRISP DC’s AD Vault. Case illustrations will be used throughout. Providers will be invited to reflect upon and address their own discomfort with Advance Care Planning. \nDescription: This case-based\, hands-on session will allow physical and behavioral health professionals to walk through the steps of engaging patients with different presentations and personalities in Advance Care Planning\, codifying their wishes in Advance Directives\, and making those Advance Directives accessible on the health information exchange. The session’s experiential design will help providers consolidate their skills\, increase their comfort and confidence\, and feel inspired to approach patients about Advance Care Planning with new ideas and tools. It will include ad hoc case discussions and time for providers to raise questions and concerns. \nSpeakers: Jean Glossa\, MD\, Barry Jacobs\, Psy.D.\, Corey Main\, MBA CRISP DC\, & & Nathaniel Curry\, DC Department of Health Care Finance \n \n  \n  \n  \nLearning Objectives: \n\nOutline processes of and barriers to Advance Care Planning\nDescribe uses of MyDirectives for uploading and accessing Advance Directives in timely fashion\nPractice synthesizing clinical skills with use of MyDirectives technological platform\n\nContent Level: Foundational \nTarget Audience: Primary Care and BH/SUD providers and their care teams. \nCourse Interaction & System Requirements: This live webinar is fully interactive.
