Transforming Maternal Health: Whole-Person Care & Payment Reform
Strengthening whole-person, value-based maternal care across the District
The Transforming Maternal Health (TMaH) Learning Collaborative, launched in May 2026, is designed for maternal health providers, including OB-GYN physicians, FQHCs, doulas, and perinatal community health workers, who are committed to strengthening their whole-person care practices to improve perinatal health outcomes. This Learning Collaborative offers various opportunities for peer learning and tailored support, including virtual group learning sessions, in-person workshops, and individualized coaching, all intended to support providers in building capacity for patient-centered, value-based care aligned with the District’s Transforming Maternal Health (TMaH) Model.
What the Learning Collaborative Offers
- Gain practical skills coordinated care using data, screening tools, and closed‑loop referrals
- Strengthen whole‑person perinatal support using integrated, team-based practices
- Deepen your understanding of value‑based payment (VBP) in maternal care
- Learn how to integrate VBP into operations and workflows
- Understand how to maximize revenue by optimizing Medicaid billing practices
What’s Included
- Monthly virtual learning sessions with CME/CE credit
- In‑person workshops including both tracks, managed care plans, and other community peers
- One‑on‑one coaching tailored to your goals and needs
Who Can Participate
The Learning Collaborative is intended for DC Medicaid-enrolled OB-GYN physicians and Federally Qualified Health Centers (FQHCs), including their clinical and operations staff, as well as doulas and perinatal community health workers.
Group learning includes track‑specific and combined sessions.

Physician Groups Track: Improve care delivery and practice operations
Track 2
Doulas and Perinatal Community Health Workers Track: Strengthen billing, Medicaid managed care plan engagement, and coordinated care
