Launching the Transforming Maternal Health (TMaH) Learning Collaborative: Peer Learning, Technical Assistance in Practice, and CrossProvider Journey Mapping

 

The Transforming Maternal Health (TMaH) Learning Collaborative webinar brings together healthcare providers, community-based organizations, doulas, perinatal community health workers, physician groups, and federally qualified health centers (FQHCs) to explore innovative approaches to improving maternal health outcomes across the District of Columbia. Hosted through Integrated Care DC and supported by the DC Department of Health Care Finance (DHCF), this session focuses on strengthening collaboration, improving care coordination, and advancing value-based maternal healthcare delivery.

Throughout the webinar, presenters from Health Management Associates (HMA) share practical strategies for implementing technical assistance, peer learning, and workflow improvements that support Medicaid maternal health initiatives. Attendees gain insight into how individualized coaching, cross-provider collaboration, and operational support can help organizations improve screening workflows, billing practices, referral coordination, and patient-centered care delivery. The session also highlights real-world examples of maternal health technical assistance already being provided to healthcare organizations and community partners nationwide.

A major focus of the webinar is journey mapping, a collaborative process designed to help providers better understand the maternal care experience from the patient’s perspective. Through interactive breakout discussions and peer learning activities, participants examine how care transitions, handoffs, referrals, and service coordination occur across different provider types and healthcare settings. These conversations surface common challenges, identify opportunities for integration, and encourage stronger collaboration between clinical and community-based maternal health providers.

The webinar also outlines the goals and structure of the 2026 TMaH Learning Collaborative, including upcoming coaching sessions, group learning opportunities, and technical assistance initiatives aimed at helping organizations strengthen maternal health outcomes and prepare for value-based care models. Participants are encouraged to identify priority goals for their organizations while learning from peers facing similar operational and clinical challenges.
